Depending on you or your loved ones care needs, an assessment of existing medication capabilities and needs may be required as part of the care service provision. If so, we can arrange to meet with you or a suitable welfare proxy to get your input. This may lead to a plan of care devised around you and your needs. At Mount Hill Care, we will work with your support network and other independent medical and social care practitioners involved with your care or that of your loved one.
